Home Care Services

GIC provides Home Care Services to the elderly and disabled. Our Registered Nurses will work with you, your entire family, and with your primary physician to create a plan of care that will meet your individual needs.Home Care Services include:

The Medicaid Innovations Waiver helps Individuals with Intellectual or Developmental Disabilities (I/DD) live a more independent lifestyle. 

Day Support (Individual/Group)

Day Supports is a group, facility-based service that provides assistance to the individual with acquisition, retention, or improvement in socialization and daily living skills and is one option for a meaningful day

Community Living and Supports

Community Living and Support is an individualized service that enables the waiver beneficiary to live successfully in his/her own home, the home of his/her family or natural supports and be an active member of his/her community.

Respite

Respite services provide periodic or scheduled support and relief to the primary caregiver(s) from the responsibility and stress of caring for the individual.

Crisis Intervention and Stabilization Supports

Crisis Services are designed to address acute symptom exacerbations for those with behavioral health disorders in an effort to either avert an inpatient crisis admission or to facilitate such an admission.

Residential Supports:

Residential Supports provides individualized services and supports to enable a person to live successfully in a Group Home or Alternative Family Living (AFL) setting of their choice and be an active participant in his/her community.

Behavioral Consultation

Crisis Services are a tiered approach to support waiver participants when crisis situations occur that present a threat to the participant's health and safety or the health and safety of others. These behaviors may result in the participant losing his or her home, job, or access to activities and community involvement. Crisis Services are an immediate intervention available 24 hours per day, 7 days per week to support the person who is primarily responsible for the care of the participant.

Community Networking

Community Networking Services provide individualized day activities that support the waiver beneficiary’s definition of a meaningful day in an integrated community setting, with persons who are not disabled.

Interpreting Services are available for the following languages:

Hmong, Laotian, Vietnamese, and Spanish.
